I'm leaning to Hank hallucinating. Hank has had grievances against Dean from a few seasons back when Rusty told him that he favored Dean because Dean was the weak one and needed protecting. Maybe he said that to Brock but I'm sure Hank knew it anyway. This would explain how the magic, giant teddy bear knew where to find Dean. And what would Hank's worst dream be --- finding his weak brother fucking <I>his</i> girl.
Doc V's reaction to this episode is very similar to my reaction to "The Inamorata Consequence" For half the episode it appeared as if Dr. Mrs The Monarch moonlighted as a common whore for the Ilumminati. Only in the after-credits did we learn that it was a dream. I suspect Hank's worst day is a hallucination from his concussion.
