I don't think it's a dream. Especially after Billy and White's (to be fair, alleged) insurance fraud stunt in Handsome Ransom, I would totally believe that Billy waltzes around with false paperwork and practices without a medical license (what a little renegade). Like how Rusty technically isn't a Doctor as he never technically graduated college.

He woke up next to Sergeant Hatred pre-rehabilitated-pedo phase and we can surmise that he'll wake up next to sexy vampire ladies on Sunday. Him waking up next to anyone else doesn't seem so out of place anymore if you ask this humble gravy.
