The Monarch was right! In Victor Echo November he stated that Phantom Limb got invisible limbs from trying to be cool like the Monarch and accelerating the growth of his muscles and in that shot was Billy!!!

Makes sense, seeing how the Monarch was one of Phantom Limb's henchmen. He may have even heard the story firsthand from PL.

As far as we know, professor Phantamos circa 1989 has never met the Monarch, and the Monarch has not met Sheila/Dr. Girlfriend, and these events are after the Monarch has left college and he's just spinning the events in the retelling to stroke his own ego. If his version is "more correct" than other people's versions of this story, that's accidental. He wasn't there and is repeating hearsay.

It think it's hardly accidental. While it's true that the Monarch wasn't there at the time, it's likely that Phantom Limb told his henchmen about his origins at some point. So Monarch was in a position to get the story directly from the man involved. Henchmen 21 and 24, on the other hand, probably pieced a story together from things they overheard from the Monarch and Dr. Girlfriend. It makes sense that their version is less accurate. Now Hank obviously didn't know any of the real story, but he may have gotten a vague impression from Brock or Peter White that Billy and Phantom Limb were somehow connected. He just made up a crazy story about the two because hey, he's Hank.

Hmm, I just remembered that 24 was a Shadowman too. Maybe he's more forgetful than the Monarch or just wasn't interested enough to get the story when he had the chance.

As you yourself admit, both the Monarch and 24 heard second-hand hearsay, both were just as "well placed"; but the Monarch just happened to hear (and repeat) the version that was closer to the truth. That's what I meant by "accidental" - the Monarch doesn't know the facts any better than anyone else does. He's repeating the version of the story that he happened to hear.

You are also assuming that Phantom Limb would be telling his henchmen his own origin story, which he might not have done. If he wasn't telling, that would have gotten the rumor mill going, thus explaining the multiple origin stories.
