This episode leaves me with a lot of questions about Billy. I'd always assumed that he was a genius, but unsuccessful due to bad luck and maybe a poor personal choices. But after watching this episode, I got the impression that we was only a little smarter than average. Maybe at the same level of intelligence as Rusty.
Did he ever go to college for real? At some point, he became a skilled surgeon(he operated on Dean's balls, gave Jonas Jr a robot hand, and reattached Rusty's severed arm). But this episode makes it look like he's just been bumming around in the trailer with White for the past 20 years.

White probably studied communications with a minor in computing, or perhaps it was the other way around.
20 years is definitely a long enough time for Billy to become a skilled surgeon...especially if that's what they told him he was studying when he "woke up" after having his memory wiped. He could have started working toward it then.
